Conclusions Atypical presentations of gynecomastia are often not recognized, with little attention to breast development in prepubertal
non-obese children. Since prepubertal gynecomastia could be a sign of possible underlying diseases, a thorough examination
and further research is recommended. If there is no causal treatment, surgical resection is the therapy of first choice. Peripheral
liposuction and surgical resection of the gland tissue are the mainstay of treatment. In summary, we describe two cases of
prepubertal unilateral gynecomastia with a normal endocrine workup. Further research is needed to establish the pathophysiologic
mechanisms of prepubertal gynecomastia, since underlying etiology in most cases remains unclear.

Launois-Bensaude syndrome is a rare pathology. The presence of multiple, symetric, non encapsulated lipomatous masses in the face, neck, upper arms and upper trunk is typical. Men are especially affected between age of 35 and 50. The disease is frequently associated with alcoholism, hepatopathy, glucose intolerance, hyperuricemia, and malignant tumors of the upper airways, requiring thorough clinical evaluation of all patients. Cosmetic deformity and compression lead patients to seek treatment. Therapy is difficult. Dietary treatment and weight loss are of limited value in the management of this pathology. CONCLUSION: Fat transfer alone can efficiently restore breast volume after mastectomy, granting all advantages related with autologous reconstruction. No donor site morbidity is present; in fact some secondary benefits are observed thanks to the correction of eventual disgraceful lipodystrophies. These secondary benefits strengthen patient compliance improving iterative procedures tolerance. Lack of available adipose tissue and high breast volume are the major morphological limits of the technique. In our experience, fat transfer appears to be a promising technique for breast reconstruction. Long term results still have to be evaluated before it can become a standard.
